Over the past 18 months, the PCN has been an early adopter in Frailty, working
closely with our local multidisciplinary team (MDT) and wider health partners to deliver
specific projects related to the 14 Frailty Interventions as part of Lincolnshire’s commitment
to improving outcomes for older people.

Building on this the PCN has been chosen to pilot a dedicated Frailty Team across our
network. The team will include three new specialist roles: Advanced Nurse Practitioner (ANP),
Occupational Therapist (OT) and Care Coordinator.
