General Practice Assistants (GPAs) - form part of a multi-disciplinary team within primary care, and provide a support role, carrying out administrative tasks, combined in some areas with basic clinical duties, helping to free up GPs time and contribute to the smooth running of appointments, improving patients experience in the surgery.
Clinical Pharmacists – answer your questions about medications and how to get the best results from them. They support the Practices with medication reviews and safety monitoring. Some of our clinical pharmacists will also work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Medacy, who are delighted to support Trent Care PCN and its member practices. We have been helping primary care organisations since 2014. All Medacy pharmacists are Independent Prescribers confident in their ability to prescribe in multiple clinical areas. Our Pharmacy Technicians have experience working across a range of practice settings and are well placed to support pharmacists and practice teams in delivering excellent care. Although our Pharmacists and Technicians work remotely, they work very closely with the practices they support. Lastly, services evolve as practice and PCN needs change, and we are always looking for better ways to support the practices we work with.
Pharmacy Technicians – liaise with Clinical and Community Pharmacists, GP Practices, and hospitals to ensure you get the correct medications at the right time. They review medications and are available to help you understand how to use your medications safely.

My name is Gemma, I am the PCN Pharmacy Technician. I have over 10 years of experience within the healthcare sector, working in Community Pharmacy and Prison Pharmacy. I currently work at the Glebe Practice, Trent Valley Surgery and Hibaldstow Medical Practice, alongside the pharmacy team to ensure safe prescribing of medication.
Care Coordinators – work with people who need extra support to navigate the health care system, helping to connect them to the right services or professionals at the right time. They act as a central point of contact to ensure support is available to help people manage their care.
We have a Care Coordinator based in each 5 of the practices within the Primary Care Network as well as our PCN Care Coordinator who is also our PCN Cancer Care Coordinator Lead covering all 5 of our practices.
First Contact Physiotherapists – are available to diagnose and treat muscular and joint conditions. They can advise on managing your condition or refer to specialist services if needed.
Our First Contact Physiotherapists are:
HS Healthcare - We provide support with musculoskeletal patient caseload management within the primary care through Physiotherapy, First Contact Physiotherapy and Advanced Physiotherapy services under various NHS contractual arrangements.
Trent Care PCN is working along side the NHS to strengthen its support for the people who live and work in and around care homes. People living in care homes should expect the same level of support as if they were living in their own home.

My name is Ricki and I’m excited to introduce myself as the new Primary Care Senior Mental Health Practitioner within the Trent Primary Care Network. I bring with me a breadth of experience across a variety of mental health settings. I began my journey in mental health as a Health Care Support Worker, gaining valuable insight and hands-on experience in supporting individuals across all areas of mental health care. In 2017, I qualified as a Registered Mental Health Nurse. Following qualification, I spent five years working with the Crisis Team, where I developed a strong foundation in crisis intervention and acute mental health support.
Social Prescribing Link Workers help connect people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing. giving you time to focus on what matters to you most.

Hi, I’m Abbie and I am a social prescriber for the Trent PCN. I have been in my role for nearly three years now – it is without a doubt, the most rewarding job I have ever had. I get to support people into the community, a community they feel a little lost in. With my help, people can attend groups and meet new people who then become friends. We attend leisure activities to help their mental and physical health, and much much more. With social prescribing, people can reduce their isolation and begin to feel part of a wonderful community again.
High Intensity Use Lead Workers support individuals who frequently attend A&E or use emergency services more than expected. This service takes a person-centred approach, building trusted relationships and offering long-term, practical support to help people access the appropriate care and improve their quality of life.

Annette is an experienced healthcare professional with a diverse background spanning community health, primary care, secondary care, and the pharmaceutical sector. Her most recent role was Health and Wellbeing Coaching within a community frailty team which allowed her to support individuals with long-term conditions, mental health challenges, and social difficulties. Annette is skilled in liaising with multi-agency professionals—including social care, health, education, and voluntary services—to ensure coordinated and effective support.
