General Practice Assistants (GPAs) - form part of a multi-disciplinary team within primary care, and provide a support role, carrying out administrative tasks, combined in some areas with basic clinical duties, helping to free up GPs time and contribute to the smooth running of appointments, improving patients experience in the surgery.
Our GPA's are;
Clinical Pharmacists – answer your questions about medications and how to get the best results from them. They support the Practices with medication reviews and safety monitoring. Some of our clinical pharmacists will also work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Medacy is delighted to support Trent Care PCN and its member practices. We have
been helping primary care organisations since 2014. All Medacy pharmacists are
Independent Prescribers confident in their ability to prescribe in multiple clinical
areas. Our Pharmacy technicians have experience working across a range of practice
settings and are well placed to support pharmacists and practice teams in delivering
excellent care.
Although our pharmacists and technicians work remotely, please include them in
communications from the practices they support so they look and feel like practice
team members. Lastly, services evolve as practice and PCN needs change, and we
are always looking for better ways to support the practices we work with. We ask for
positive or constructive feedback so we can continue improving the service we
provide.
Pharmacy Technicians – liaise with Clinical and Community Pharmacists, GP Practices, and hospitals to ensure you get the correct medications at the right time. They review medications and are available to help you understand how to use your medications safely.

Care Coordinators – work with people who need extra support to navigate the health care system, helping to connect them to the right services or professionals at the right time. They act as a central point of contact to ensure support is available to help people manage their care.

I’ve always loved working in roles where people are at the heart of everything we do, and knowing that the work we do each day is making a difference to someone’s life is what motivates me. I’ve previously worked in the NHS as a Level 2 Pharmacy Assistant, and most recently as a Veterinary Receptionist. In my role as Care Coordinator, I’ll be working across the PCN to support the ARRS workforce, the new Age Matters team, and to strengthen the PCN’s connections with wider community services. My aim is to help each practice and their patients access the care and support they need.
First Contact Physiotherapists – are available to diagnose and treat muscular and joint conditions. They can advise on managing your condition or refer to specialist services if needed.

Trent Care PCN is working along side the NHS to strengthen its support for the people who live and work in and around care homes. People living in care homes should expect the same level of support as if they were living in their own home.

Health and wellbeing coaches (H&WBC) - support people to increase their ability to self-manage, motivation levels and commitment to change their lifestyle. They are experts in behaviour change and focus on improving health related outcomes by working with people to set personalised goals and change their behaviours. They work with people with physical and/or mental health conditions and those at risk of developing them.

My name is Paul Bailey, I am a health and wellbeing coach for Trent PCN, I work at three different surgeries which include, Trent Valley, Glebe Practice and Hibaldstow surgery.
I see patients either face to face at the surgeries or via phone call, patients can be referred to me via the surgery itself. I see patients for a host of different reasons covering many aspects of health and wellbeing, this can range from nutrition, exercise, anxiety, bereavement, loneliness, mental health, long term health conditions and many more.
As a health coach I have more time with my patients. Patients can have sessions taking up to 30-45 minutes and these can be ongoing for up to 6 sessions. We work on what is important to our patients, we are here to help and support our patients to improve and move in the right direction by making small achievable changes that feel realistic and comfortable to them.
Hi my name is Shelly Wilkins. I am a health and wellbeing coach at Caskgate Street and Cleveland surgeries in Gainsborough. The service is free to registered patients and can be referred through any of the clinicians or GPs.
What I do? I help empower people who would like to make a lifestyle or behavioural change around their physical or mental wellbeing. These can include the following.
- Managing low mental health conditions.
- Lifestyle changes around health, weight and moving more.
- Improve self esteem and confidence.
- How to manage long term health conditions.
- How to manage and improve chronic pain conditions by improving lifestyle changes.
Mental Health Practitioners – perform assessments, provide advice and support to manage your condition. They can support access to other community mental health resources.

Social Prescribing Link Workers help connect people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing. giving you time to focus on what matters to you most.

Hi, I’m Abbie and I am a social prescriber for the Trent PCN. I have been in my role for nearly three years now – it is without a doubt, the most rewarding job I have ever had. I get to support people into the community, a community they feel a little lost in. With my help, people can attend groups and meet new people who then become friends. We attend leisure activities to help their mental and physical health, and much much more. With social prescribing, people can reduce their isolation and begin to feel part of a wonderful community again.
